Filling Out Forms for Social Security
The Social Security Administration has many forms for you to complete while they process your application for disability benefits. These include forms about your medical treatment, work history, and day to day functioning. The Social Security Administration uses generic forms that they send out in every case, regardless of what your disability is. Frequently, these forms do not ask questions that relate to you or your disability. In our experience, when you answer these forms, you should provide them with the details of your disability regardless of whether the form asks for that information.
